Is There a Link Between Premature Ejaculation and Erectile Dysfunction?

Studies on PE & ED

There’s a strong correlation between PE and ED. A study reviewed papers from 1996 to 2024 on erectile dysfunction and premature ejaculation and here's what they found:

  • About 30% of men with erectile dysfunction also have PE.
  • About 50% of men with PE also have ED.

From my experience with clients, I can confirm this.

At least 50% of guys who come to me with premature ejaculation also have some form of erection problems. It might be losing erection mid-sex, right before penetration, or not getting it up at all. The causes vary — sometimes psychological, sometimes physical or genetic, and often a combination.

Causes of Erectile Dysfunction

Let’s talk about the causes of erectile dysfunction first.

  • Age: The older you get, the less reliable your erection will be.
  • Psychological causes: Stress or anxiety play a huge role. Anxiety releases adrenaline and norepinephrine, which make your erection collapse.
  • Diseases and medications: Certain diseases and side effects of medications can cause ED.
  • Drug abuse: Yeah, the party stuff can tank your performance over time.
  • Venous leak: A physical glitch where blood doesn’t stay in your penis long enough to keep it up.
  • Partner mismatch: TOO OFTEN overlooked. Sometimes it’s your partner. Maybe you don’t find her sexually attractive, or there’s a mismatch in chemistry. In Germany, we say, “I can’t smell him.” If your partner has stressful energy or rejects sex often, it can cause anxiety and make erections collapse.

The approach is to find the cause and remove it if possible. If not, drugs or other solutions can help. I’ve worked with all kinds of clients, and it’s always individual. Sometimes it’s psychological and can be removed quickly. Sometimes it’s age or genetics, and you might need cock rings or ED drugs — even a microdose can be enough.

Medication for PE can be problematic because it often improves PE but worsens ED and lowers sex drive.

Sometimes, erectile dysfunction can indirectly lead to premature ejaculation. Guys with ED might overcompensate with stimulation to get hard, which accelerates orgasm and causes PE.

Causes of Premature Ejaculation

Now, let’s talk about the causes of PE. We need to distinguish between lifelong and acquired premature ejaculation:

  1. Lifelong PE: This is usually genetic—stuff you’re born with. Causes include:
    • Sensitive glans (the head of your penis).
    • Lower serotonin levels.
    • Hyperarousability (you get turned on way too easily).
    • You can’t change your genes, but that doesn’t mean you have to accept short lasting times. There are ways to last longer. I’m an example myself.
  2. Acquired PE: This hits later in life, triggered by:
    • Medication side effects.
    • Drug abuse.
    • Going off antidepressants.
    • Diseases like prostatitis.
    • In general, acquired PE is often "curable", if you find and remove the cause.

ED can lead to PE - indirectly

Here’s a curveball: ED can actually trigger PE. Here's how. Sometimes, guys with ED need to overcompensate with visual or physical penis stimulation - just to get or stay hard, which in turn accelerates orgasm and can lead to PE.

Two Steps to Fix PE and ED

If you have both premature ejaculation and erectile dysfunction, here’s what to do:

  1. Find the cause. If you find it, you can often remove it. For acquired PE, it’s sometimes easy to remove. For lifelong PE, you can’t change your genes and "remove the cause", but you can work around it very effectively.   2. Work on it. Remove the cause if possible. If not, find a way to have great sex anyway. In every case, there’s a solution to have amazing, long sex with a great erection. It’s individual, but it’s possible.
  3. Here’s the mindset: there’s a solution for every case. No matter your mix of PE and ED, you can have amazing, long-lasting sex. It’s about finding your fix.
