Is Lifelong Premature Ejaculation Permanently Curable?

Lifelong premature ejaculation is when you orgasm in less than 1-2 min since your first sexual experience and can't control it no matter what you do.

Naturally you ask yourself if it will ever go away on its own, if it can be cured or if it's something permanent you have to live with.

In the last 10 years I've helped hundreds of guys overcome their PE and ED and can give you an experience based answer, as realistic as it can get, no fluff, no sugar coating.


Can it be cured? — It depends on the cause


First it's important to understand the possible causes of early ejaculation.

PE can be acquired or lifelong and can originate from a variety of different factors like relationship problems, genetics, diseases etc. Some of them you can change, others not.

A "permanent cure" is possible, if your lifelong PE is caused by a removable cause, such as chronic prostatitis 1Kim S. W. (2011). Prostatic disease and sexual dysfunction. Korean journal of urology, 52(6), 373–378. Accessed June 8, 2022. https://doi.org/10.4111/kju.2011.52.6.373 , a shortened frenulum or similar. If you remove it, then yes - your premature ejaculation can be "cured" in that sense.

But if none of such causes apply and it’s a just case of a genetic lifelong (primary) premature ejaculation, then you can't "cure" it, because there is no way to change your genes.

It’s then just a purely genetic predisposition for short lasting time, just like the tendency to obesity can be purely genetic

Some people are always hungry by nature, no matter how much they eat, while others are satisfied with a sandwich. Hunger  is strictly regulated by hormones like ghrelin and leptin, which are determined by your genes - which you can't simply change permanently.

And the same goes for stamina - some men can last longer in bed by nature, others don’t - by nature. However, you can still massively increase your stamina, even if PE is purely genetic and lifelong in your case. You just have to apply the right methods.

Ok, back to the initial question. So can PE be cured? As I said, Yes, if the cause is remediable, such as a disease, then premature ejaculation can be cured. Read more about the different causes of premature ejaculation here.

But if the reason for your premature ejaculation is your genetic make up, and there is no underlying disease or anything else, then there is nothing to "heal", it’s just your genetics.

And again - that doesn't mean you have to accept it. Just like a person who’s genetically prone to obesity can still achieve their ideal weight, so can you massively increase your stamina, despite genetic predisposition for short lasting time.

What else can help (not cure) lifelong premature ejaculation


In this chapter I'll provide a brief overview of the possible solutions which can be used to alleviate lifelong PE somewhat, but have their own drawbacks. Usually it's either low success rate or loss of erection.

There are also some DIY solutions you can try at home (listed below), just don’t count on them for a ‘cure’ and brace yourself for strong negative side effects.

Medicine 💊

Let's be clear: PE can't be cured by medicine. And there's no single best drug that could be recommended as "a cure". All meds have side effects, so by using medicine, you rather exchange on set of problems for another. With rare exceptions.

Prostate Drugs

As mentioned before, prostate drugs can "cure" premature ejaculation, if chronic prostatitis was the single cause for your PE. For premature ejaculators with a healthy prostate they won't do anything.

Painkillers & Antidepressants

Most antidepressants and some painkillers are SSRIs (selective serotonin reuptake inhibitors) and SNRIs (serotonin–norepinephrine reuptake inhibitors). They work by increasing serotonin in the brain and result in delaying ejaculation in men.

Common off-label drugs used for premature ejaculation are SSRIs like Zoloft (Sertraline), Lexapro (Escitalopram), Paroxetine (Aropax, Paxil), Prozac (fluoxetine) and SNRI painkillers like Tramadol (Ultram, Conzip).

They all increase the time one needs for orgasm, but also have these nasty side effects like  loss of libido (sex drive), loss of erection, apathy, drowsiness, lethargy, and nausea.

The only exception is the short acting SSRI Priligy (Dapoxetine). This drug however isn't popular either, because it extends the lasting time only by a few minutes and still has unpleasant side effects like headaches, nausea and feeling unwell in general, making sex unenjoyable.

ED Drugs

Male sexual enhancement drugs like Viagra (Sildanefil), Cialis (Tadalafil) and Levitra (Vardenafil) are primarily used for impotence

Some people think ED drugs can also help with premature ejaculation, but studies show that the ejaculation delaying effect is very minor (approx. 20 seconds).

Exercises 💪

There's regular workouts & lifting weights and there are exercises specifically targeted for PE. Unfortunately none of these will fix primary premature ejaculation for you, but there are still great benefits in doing these.

Regular exercise and workouts

  • 🏋️‍♀️    Practice squats and deadlifts in the gym, if you want to ensure having a have a rock-hard erection. They'll improve your blood supply in the lower body region and increase your testosterone.
  • 🏃🏼‍♂️    Do aerobic exercises like jogging and running to rebalance your central nervous system and improve your ejaculation control. Running more so than jogging though, since it's a faster, higher-intensity aerobic exercise. Anecdotal reports and my own experience confirm this effect. I fucking love running.
  • 🧘‍♂️    Yoga exercises on the other hand can help you balance your physical energy and make you more relaxed, which is very helpful if you want to last longer in bed.

Exercises targeted for delaying ejaculation

Most of commonly recommended exercises are useless for PE and they can't cure anything. Let's see what works and what doesn't.

  • "Start stop method" aka "Edging": with this exercise, you simply start masturbating ("START") and then, right before reaching orgasm, you stop ("STOP"). After the urge to ejaculate subsides, you continue. Widely recommended but useless ❌. 6 Premature ejaculation: A review by Sukumar Reddy Gajjala and Azheel Khalidi  
  • Kegel exercises: with this exercise, you simply train your pelvic floor muscles, and then, during sex, when you need to prevent your ejaculation, you tighten those strong muscles and avoid orgasm. In theory you should be able to resume and last longer, but in reality you then either just orgasm or lose erection. Widely recommended but useless  ❌.
  • Reverse kegel exercises: With reverse kegels you train to relax your pelvic floor muscles, which can take off the edge of your arousal during sex, so you can extend your lasting time. It works 👍.

Foods  🍽

There are no fruits or vegetables or other food you can eat to prevent premature ejaculation - let alone cure it.

Neither banana, nor lemon juice, nor ginger, garlic and honey, pumpkin seeds, coconut oil or the infamous olive oil will ever do anything for you if you're trying to last longer in bed. 

The only food that you will benefit from lots of animal products high in healthy cholesterol and l-tryptophan. Such as fatty beef steaks, fish cheese and eggs. 

Don't expect to last long in bed like a pornstar just because you ate a fatty steak.

But the cholesterol from fatty meat will definitely boost your testosterone and the l-tryptophan in it will optimize your serotonin levels. These are crucially important for overall sexual energy.

What about alcohol 🍺?  Well, it's not food, but falls under this category. It won't cure your premature ejaculation, but since it affects GABA receptors, it may slightly delay your orgasm. However, it’s very likely to destroy your erection (common problem with alcohol), so go easy on it.

Supplements  💊

Unfortunately there's a lot of misinformation on the web about supplements that supposedly work for premature ejaculation. Some of them are even recommended as a "cure", which is nonsense. 

None of these will have any impact on your lasting time. Let me list most commonly searched and recommended ones and comment on each:

  • 5-HTP - this supplement supposedly increases your serotonin. However the serotonin is increased in the gut, not in the brain, thus the generated serotonin can't pass the blood-brain barrier and do its ejaculation delaying job. I tried it many years ago, others tried it. Useless.
  • Zinc - zinc can only help you optimize testosterone if you're deficient. It will do nothing for your PE.
  • Vitamins: neither taking vitamin B12, nor vitamin E has ever worked for anyone to delay ejaculation. No supporting studies, no success reports.
  • L-Arginine and Ginseng: L-arginine and Ginseng are overhyped supplements which in theory should help with erection by increasing levels of nitric oxide, which helps relax blood vessels and improve blood flow to the penis. But in reality it just results in expensive urine. Neither l-arginine nor ginseng will do anything really for real ED and most certainly won't cure PE because they don't affect serotonin levels or penile sensitivity sufficiently. 
  • Wellman from Vitabiotics: it's nothing more than a mix of ingredients like Siberian Ginseng, Co-enzyme Q10, vitamin D, vitamin E, vitamin C, vitamin B6, folic acid, vitamin B12, biotin, magnesium, iron, zinc, copper and selenium. This mix may seem healthy, and it may be, but none of these ingredients has ever been shown to help with PE, let alone cure

While there are some natural otc remedies for PE you can use, they're very ineffective 😡.

Numbing agents 🥶

Numbing agents found in delay sprays and creams can make your glans penis less sensitive, so you feel less and last longer in bed.

Yes, it works, however you run the risk of losing your erection, since your penis will feel "frozen". This is why they're rather unpopular among premature ejaculators. 

You also won't be able to have sex spontaneously. You will have to apply them 15 minutes before sex, wash them off (otherwise your partner's private parts will get numb as well) and only them have sex.

So, it's a really inconvenient short term solution with a high risk of losing erection and is certainly not a cure.

Numbing condoms. You can also use the numbing condoms with 5% benzocaine gel on the inside. They are way less numbing than the sprays and creams (which contain anesthetics like lidocaine and prilocaine), but are also less likely to give you erection problems.

NoFap (abstinence) 😤

On reddit you'll find people claiming nofap causes premature ejaculation, while others will say PE can be permanently fixed by it. Here's the truth. Nofap is just abstaining from masturbation and orgasm and the longer you go without ejaculation, the more "sexual pressure" you'll build up. Thus, after a long period of abstinence you're very likely to have reduced control about your ejaculation. In a 7 day study no connection between IELT (listing time) and PE could be found. 7 Abstinence from Masturbation and Hypersexuality Felix Zimmercorresponding author and Roland Imhoff. link

On the other hand nofap will lead to a build up of tremendous sexual power which in turn helps with erection.

Surgical treatments 🔪

There are 4 main types of surgeries which claim to cure premature ejaculation:

  • Low & Tight circumcision with frenulum removal -  the idea is to make your penis less sensitive. While good in theory, unfortunately, studies show that it doesn't work. Circumcised men last only 1-2 min longer in bed than uncircumcised men. 8Yang et al., Circumcision does not have effect on premature ejaculation: A systematic review and meta-analysis. PubMed
  • Denervation of the glans - this surgery is marketed as a cure for PE, however it doesn't deliver. The additional lasting time ranges between 1.1 minutes to 3.8 minutes.  9 G.‐X. Zhang et al. Selective resection of dorsal nerves of penis for premature ejaculation link
  • Glans penis enlargement - the idea is to make the glans larger through a hyaluronic acid injection, which should result in reduced sensitivity. However the effect is negligible (+1.5 to 2.5 min) and disappears after a few months  10T I Kwak et al Long-term effects of glans penis augmentation using injectable hyaluronic acid gel for premature ejaculation link
  • Frenuloplasty - this is the only surgery that can cure your PE if the single reason for it was a shortened frenulum. During the frenuloplasty procedure your shortened frenulum will be lengthened or removed all together and normal ejaculation control can be achieved. Shortened frenulum (frenulum breve) is a very rare cause for PE though, I wouldn't have too high hopes on that.

There is also a myth that a vasectomy can cure premature ejaculation. No, it can't. All it does is permanently block the sperm-carrying tubes, preventing sperm from reaching the semen that is ejaculated from the penis, having no impact on ejaculation control whatsoever.

Snake Oil "cures" 😡

There are a lot of treatment options which often claim to be the cure for PE but they don't improve it even in the slightest. Avoid these approaches if you want to save your time and money:

All of these practices are completely ineffective for PE. There are no supporting studies and no success stories.


Best natural way to cure premature ejaculation 🤗


While there's time and place for remedies like numbing agents, antidepressants and surgeries, they all share two common problems: 1) erection problems and 2) high risks:

  • 🥶 Numbing agents reduce sensitivity and slow down your orgasm, but with a frozen penis you'll struggle maintaining erection 👎.
  • 💊 Antidepressants raise your serotonin and delay ejaculation, but reduce your sex drive and make it difficult to stay hard 👎.
  • 🔪 Surgeries like the glans denervation may reduce penile sensitivity, but are risky, costly and can result in permanent erection problems 👎.

How many days will you need to "cure" premature ejaculation?
